Primary Care Community Officer.

Hosted by Norfolk Citizens Advice, Working in the community for the Norwich PCN.

My current role is to develop relationships across the health & social care sector, in line with a framework, called the PCN maturity matrix. I need to develop the framework through community & stakeholder engagement, ensuring KPI’s are met and creative thinking is applied to key activities and ideas. I look to identify gaps, challenges, opportunities, review our operating process’s, developing relationships, mapping the current community health and social care offer, help to tackle inequalities and explore how we can support the wider determinates of health better.

I have been working alongside NIHA for over a year now, supporting their work, extending into our communities, This partnership has seen some exciting projects grow, but also new relationships develop.

Its an exciting future and looking forward to how we can connect our communities further, through research, outcomes, developing partnerships and support.
